Let There Be Art (LTBA) – Consent and Acknowledgment
By enrolling my child in any programs, classes, workshops, camps, events, or activities offered or operated by Let There Be Art (“LTBA”), whether conducted independently or in partnership with the City of Vacaville or any other organization, I acknowledge and agree to the following:
Independent Agreement
I understand that while registration for certain LTBA programs may occur through the City of Vacaville or another third-party provider, this Consent and Acknowledgment is a separate agreement that applies specifically to LTBA and its operations. It is in addition to, and does not replace, any agreements required by the City or other hosting organization.
Participation and Assumption of Risk
I acknowledge that participation in LTBA activities involves inherent risks, including but not limited to the use of art supplies and tools, physical movement, group participation, and possible off-site or outdoor activities. I voluntarily assume all risks associated with my child’s participation and agree to release and hold harmless LTBA, its owners, employees, instructors, contractors, and affiliates from any and all claims, liabilities, damages, or expenses arising from participation, except where prohibited by law.
Medical Authorization
I certify that my child is physically able to participate in LTBA activities. In the event of an injury or medical emergency, I authorize LTBA staff to obtain necessary medical treatment for my child and agree to be responsible for any resulting costs.
Allergies, Materials, and Special Considerations
I understand that LTBA programs may involve the use of various art materials, including but not limited to paints, adhesives, clay, and other supplies. I agree to inform LTBA in advance of any allergies, sensitivities, medical conditions, or special needs my child may have. LTBA will make reasonable efforts to accommodate disclosed needs but cannot guarantee an allergen-free environment.
Authorized Pick-Up
I understand that children will only be released to a parent/guardian or individuals authorized by the parent/guardian. I agree to provide accurate and updated pick-up information and acknowledge that a valid photo ID may be required for authorized individuals. LTBA is not responsible for children left unattended beyond designated program hours.
Code of Conduct
I understand that participants are expected to follow all rules and behave respectfully toward staff and other participants. LTBA reserves the right to remove any participant for unsafe, disruptive, or inappropriate behavior, without refund.
Cancellation and Refund Policy
I understand that LTBA may establish cancellation and refund policies that vary by program. Unless otherwise stated, refunds may not be provided for missed classes, participant withdrawal, or dismissal due to behavior. In the event LTBA cancels a program, a refund or credit may be issued.
Photo and Media Release
I grant Let There Be Art permission to photograph, record, and use my child’s likeness, artwork, and voice in any LTBA-related promotional, educational, or marketing materials. This includes, but is not limited to, use in social media, websites, advertisements, email communications, and printed materials such as flyers and brochures.
I understand that this release applies to both programs conducted in partnership with the City of Vacaville and those conducted independently by LTBA, and is separate from any media release granted to the City. I acknowledge that no compensation will be provided and that all such media shall remain the property of LTBA.
Acknowledgment of Agreement
By enrolling my child or allowing participation in LTBA activities, I confirm that I have read, understand, and agree to the terms outlined in this Consent and Acknowledgment.
